Attackers are using these extension spoofer tools to fools the vicim simply spoofing the extension of malicious applicaiotn/payload.
E.g:
if your payload/application name is: Sunny_Leone.exe
after spoofing it will become "Sunny_Leone.jpg"
Step:
1.Click on Browse
2.Add desired extension
3.Click on save.
4.Done !
